DOBSIL has a comprehensive mode of action. DOBSIL acts on the endothelial layer and basement membrane of the capillaries. DOBSIL increases endothelial nitric oxide levels by enhancing the activity of nitric oxide synthase, decreasing capillary hyper-permeability. The histamine and bradykinin induced hyperpermeability and capillary fragility is reduced and red blood cell membrane flexibility is increased. It is used in the treatment of Diabetic Retinopathy,Haemorrhoids,Chronic venous diseases.
DOBSIL shows antiplatelet and fibrinolytic activity by inhibiting platelet activation factor (PAF) and enhancing the release of tissue plasminogen activator (tPA), thereby improving the local blood flow to tissue, otherwise inhibited due to thrombosis.
DOBSIL also inhibits the two pathophysiological reactions in diabetes via polyol pathway and glycation of proteins due to its inhibitory effects on aldose reductase. DOBSIL may also inhibit the formation of sorbitol, thus providing another possible mechanism for its usefulness in diabetic retinopathy.
